A Game-Changer for Type 2 Diabetes: Developing the New Hybrid Care Path

In 2023, i-mens, in collaboration with partners, completed a groundbreaking clinical study (RCT) exploring a hybrid care model for managing type 2 diabetes. This innovative approach, which combines patient self-monitoring with continuous remote monitoring and just-in-time interventions, aims to revolutionize diabetes care, making it more effective and cost-efficient.

Using smart devices connected to a validated app (HomeHealth), 50 patients regularly measured key health parameters, including blood glucose, blood pressure, and physical activity. These data points were monitored 24/7 by nursing staff of Z-plus, a telemonitoring center that provided immediate remote support when values exceeded pre-set thresholds. 

To demonstrate that the health of people with diabetes has improved through the hybrid care pathway, a significant difference in HbA1c levels between the start and end measurements in the intervention group had to be observed. While the results so far have shown only partial significance in this group, the study supports further evaluation over a longer period. However, early findings have already demonstrated meaningful improvements in body weight trends, glucose levels, and a reduction in interventions per patient throughout the project.

In terms of patient satisfaction, 90% of participants in the intervention group indicated a preference for the new care pathway. Additionally, there was a clear positive shift in patients' motivation to monitor their health, as well as an increased sense of control and safety.

These promising results lay a strong foundation for expanding the project to include hospitals and general practitioners in a future “2.0 version.”

Expanding the hybrid care model

The hybrid care pathway aims to empower patients through self-management and reduce the long-term complications associated with diabetes, easing the burden on the healthcare system. By combining patient-driven data collection with professional monitoring and personalized interventions, this project offers a scalable, efficient alternative to traditional care models.

Project highlights: Revolution in diabetes care

  1. Patient self-management and prevention. Through the use of smart devices such as glucose meters and activity sensors, patients actively monitor their own health. The data are then transmitted via the HomeHealth app, allowing patients to gain insights into their condition while enabling healthcare providers to track progress in real time.
  2. Continuous monitoring and tailored interventions. Z-plus care center operators monitor patients’ health data 24/7 through Comarch e-Care platform. When any parameters exceed personalized thresholds, the center intervenes by contacting the patient and, if needed, escalates the case to relevant healthcare providers, including doctors and diabetes educators.
  3. Effectiveness and cost-efficiency. A key focus of the project is evaluating the hybrid care pathway's effectiveness compared to standard care models. Medical parameters such as HbA1c levels, weight, and blood pressure are measured alongside cost-effectiveness. In addition, i-mens is conducting a willingness-to-pay study, which will help determine future pricing models and ensure the sustainability of the approach within the national healthcare system.

Looking ahead: The future of hybrid care

The final report from the clinical study is available since April 2024 and the preliminary data show that the hybrid care model offers a viable alternative for managing diabetes. The next phase of the project, involving hospitals and general practitioners, will further refine the approach to ensure its integration into standard care protocols.

We are optimistic about the future of this hybrid care pathway and its potential to improve the quality of life for patients with type 2 diabetes. With the combination of just-in-time, remote interventions and software such as that provided by Comarch, we believe this project can serve as a model for chronic disease management in Belgium and beyond, added Dorien Vandormael from i-mens.
